Olfactory neuroblastoma: a case report.

- Abstract
Introduction: Olfactory Neuroblastoma, also known as esthesioneuroblastoma (ENB), is a rare malignant neoplasm that affects the olfactory neuroepithelium and is responsible for 3 to 6% of all malignant nasosinusal tumors. There are few reports in the literature and there is no consensus on its management Case report: A 41 years old male, seeks the emergency complaining of persistent headache, paresthesia in the hemiface, right otalgia and episodes of epistaxis. On physical examination only lymphadenopathy in the right cervical was observed. Videonasolaryngoscopy has shown an infiltrative lesion in the region of the right sphenoethmoidal recess. Computed tomography (CT) exhibits an expansive and infiltrative lesion in the posterior ethmoid cells with sphenoid extension and lymph node enlargement in the cervical chain. No significant changes were presented in the staging chest and abdomen examination. PET/CT confirmed a lesion and it's extending to the base of the skull with cervical lymph nodes with hypermetabolism compatible with secondary implants. A fine-needle puncture of the lymph node revealed a cytological picture compatible with carcinoma metastasis. The patient underwent endoscopic resection and the anatomopathological and immunohistochemical tests showing findings compatible with ENB. Chemotherapy treatment was started with significant clinical improvement after the first cycle. At the present date, the patient awaits for radiotherapy sessions. Conclusion: ENB is a rare condition and its approach is not a consensus. The use of imaging tests and the anatomopathological and immunohistochemical study are important tools in the management of these cases.
