- Abstract
Chest radiography: lateral view shows the posterior course of the lead (arrow) as it travels from right to left atrium before crossing the mitral valve into the left ventricle. Both 2D and reconstructed 3D images show the lead crossing a 1.4 cm × 0.7 cm secundum atrial septal defect and mitral valve before its final position within the left ventricle Although rare, inadvertent LV lead implantation across an atrial septal defect during device implantation can be avoided either by (1) recognizing that ventricular paced QRS complexes have a RBBB morphology or (2) checking fluoroscopy in the left anterior oblique projection for a posterior ventricular lead position [[3]].
