- Abstract
Die Handgelenksarthrographie erscheint als Screeningmethode zur Erkennung von Läsionen des TFCC und von intrinsischen Bandverletzungen bei der Radiusfraktur an typischer Stelle durchaus geeignet, insbesondere da die Untersuchung nur einen minimalen zusätzlichen Aufwand erfordert und in einer Hand und unmittelbar mit der Versorgung der eigentlichen Fraktur erfolgen kann. Aufgrund unserer eigenen Erfahrungen und der doch beträchtlichen Zahl an Zusatzverletzungen bei Radiusfrakturen an typischer Stelle hat sich in unserem Haus unser diagnostisches und therapeutisches Vorgehen bei jungen Patienten geändert: 1. Bei Patienten mit geplanter operativer Versorgung einer Radiusfraktur loco typico (unabhängig von der Art des operativen Eingriffes) wird vor der Operation eine arthrographische Abklärung in der beschriebenen Form und Technik durchgeführt. 2. Bei Patienten unter dem 40. Lebensjahr wird wenn möglich, und besonders bei begründeter Verdachtsdiagnose im Ü bersichtsröntgen (Erweiterung des scapholunären Gelenksspaltes, Disi-, Pisi-Fehlstellung, großes dorsales und ulnares Schlüsselfragment mit entsprechender Dislokation, Auslaufen einer Fraktur am Radiusfirst) in Lokal anaesthesie eine Arthrographie unter Bildwandlerkontrolle durchgeführt. 3. Vor jeder Handgelenksarthroskopie wird obligat eine Arthrographie durchgeführt.
