- Abstract
A 36-year-old woman with idiopathic pulmonary arterial hypertension (PAH) and end-stage heart failure underwent a balloon atrial septostomy (BAS) procedure, which significantly improved her symptoms. Cardiac MRI showed reverse cardiac remodeling, with increased left ventricular filling, increased cardiac output, and decompressed right heart chambers. The MRI also revealed a significant right-to-left shunt. BAS appears to be an effective treatment for severe PAH and end-stage heart failure, and cardiac MRI can provide valuable insights into the changes in cardiac structure and function following the procedure.
