Effect of patient-centered transitional care services on clinical outcomes in patients hospitalized for heart failure: the PACT-HF Randomized Clinical Trial. Effect of a hospital and postdischarge quality improvement intervention on clinical outcomes and quality of care for patients with heart failure with reduced ejection fraction: the CONNECT-HF Randomized Clinical Trial. Heart failure (HF) is a leading cause of mortality, morbidity, and hospitalization in older adults.[1] The combination of four classes of guideline-directed medical therapies (GDMTs) results in >70% reduction in mortality in patients with HF with reduced ejection fraction (HFrEF), yet translating this evidence into practice remains a challenge.
