- Abstract
INTRODUCTION: Gallstone ileus is a rare complication of cholelithiasis, mostly in the elderly. It accounts for 1%-3% of mechanical bowel obstruction and is associated with high morbidity and mortality. We present our experience in managenet gallstone ileus and discuss about surgical approach. METHOD: We carried out a retrospective study of all gallstone ileus cases that underwent surgery in our department during the last 10 years. We took into consideration diagnostic elements, time from admission to surgery, type of surgery and post-operative outcome. RESULTS: Fourteen patients were included in the study, nine females (64%) and five males (36%) with median age 69. Diagnosis was suggested by clinical features of acute or incomplete intestinal obstruction; it was sustained by imagistic studies with different degrees of relevance. The average time from admission to surgery was 5.6 days. Surgical approach varied from simple enterolithotomy to additional cholecystectomy and fistula repair. CONCLUSION‟s: Bilary ileus is a pathology of patients of advanced age with important comorbidities and must be suspected when symptoms of occlusion are present, regardless of gender. Early diagnosis is the most important for potentially successfully treatment. The best choice for surgical procedure in our experience is enterolithotomy.
