- Abstract
Objective: To evaluate the improvement of patients with overactive refractary bladder, after the administration of botulinic toxin A. Material and methods: An experimental, prospective, longitudinal, comparative, cohort, open study was done. Twelve patients with overactive refractory bladder were selected, a pretreatment cystogram, cystoscopy and urodynamic study were performed. Likewhise and Spanish version of the Bladder Control Self-Assessment Questionnaire was applied. Under sedation 200 U of botulinic toxin A were applied, uniformly distributed among 30 application sites at 2 mm of depth of the bladder muscle. A followup was performed for a period of six months. Results: Using the Friedman test we find that the median bladder capacity increased from 265 to 350 cc; the detrusor pressure lowered from 39.50 to 17.50 cm of HO; the bladder overactivity changed from 6.50 to 2.50 vesical contractions; the residual urine from 22.50 to 72.50 mL; the symptoms decreased from 12 to 3.50; the bothers from 12 to 3; using the χ² test the improvement in the intake of anticholinergics was proven, 52% of patients lowered their dose, and 16.6% withdrew it; all data with a p lower than 0.05. At six months after the injection, return of urodynamic values to pretreatment scores was observed. Conclusion: Botulinic toxin A is an effective treatment for the overactive refractory bladder.
