- Abstract
Objective To investigate the formation situation of different grades of cleavage stage embryo on the 3rd day(D3) culturing to blastocyst on the 6th day(D6)and to analyze the influencing factors of formation rate and formation velocity for the non-fine embryo continuously culturing to blastocyst. Methods 180 couples undergoing the assisted pregnancy by assisted reproductive technology in the reproductive center from January to August 2017 were selected. A total of 1 009 remaining embryos after D3 cleavage stage freezing were divided into the high quality embryo group(n=664)and non-high quality embryos(n=345). Then the total blastocyst formation rates were compared between the two groups. Meanwhile they were divided into the non-blastocyst formation subgroup,the 5th day(D5)subgroup,D6 subgroup and D5+D6 subgroup according to the blastocyst formation velocity,and divided into the group A,B and C according to the blastocyst formation situation. Then the influencing factors of blastocyst formation rate and formation velocity were analyzed. Results The total blastocyst formation rate in the high quality embryo group and the high-quality blastocyst formation rate in the D5,D6 and D5+D6 subgroups were superior to those in the nonhigh quality embryo group,the differences were statistically significant(P<0.05). Compared with the group A,the constituent ratio of high quality embryos at cleavage stage,long scheme constituent ratio,constituent ratio of in vitro fertilization and E2 level on HCG day in the group B and C were higher,while the constituent ratio of antagonist scheme and constituent ratio of oocyte intracytoplasmic single sperm injection were lower,and the differences were statistically significant(P< 0.05). Compared with the non-blastocyst formation subgroup,the constituent ratio of high quality embryos at cleavage stage and E2 level on HCG day in the D5,D6,D5+ D6 subgroups were higher,while the constituent ratio of non-high quality embryo at cleavage stage and ovulatory obstacle factors were lower;the long scheme constituent ratio of the D6,and D5+D6 subgroups was higher and the antagonist scheme constituent ratio were lower,while the constituent ratio of fallopian tube factors in the D5 subgroup was higher,the differences were statistically significant(P<0.05). Conclusion The D3 non-high quality embryo still has the potential to develop to the blastocyst,but the formation rate and planting rate of high-quality blastocyst are lower than those of high-quality embryo at cleavage stage. Medication regimen,insemination method and infertility causes affect the total formation rate of blastocyst. The total formation rate of blastocyst is high and the cycle embryo implantation rate is much higher.
