The article highlights flaws in a hospital's risk assessment process revealed by a Mental Welfare Commission for Scotland, citing the case of a patient who killed a man while he was in the community on conditional discharge. The incident shows the lack of any systematic approach to risk assessment and management, and that there was no agreed plan of how clinical staff and services should respond in dealing with people with mental disorder.
