Factores sociales determinantes de la mortalidad materna en Colombia, 2016.

- Abstract
Introducción. La mortalidad materna tiene una estrecha relación con las inequidades en términos de acceso a los servicios de salud y con las brechas entre ricos y pobres, y es un grave problema en salud pública en los países en desarrollo, sobre todo porque, en gran medida, es evitable. Objetivo. Establecer los factores sociales determinantes en las muertes maternas ocurridas en Colombia durante el 2016. Materiales y métodos. Se hizo un estudio descriptivo retrospectivo con los datos de la notificación al Sistema de Vigilancia de Mortalidad Materna en web correspondientes al 2016, incluida la información de 100 casos con análisis completo. Se calcularon las frecuencias absolutas y las relativas, y se hicieron los análisis bivariados utilizando el programa Excel. Resultados. La mayor razón de mortalidad materna se registró en mujeres mayores de 34 años. El 24 % presentaba baja escolaridad; en el 49% el ingreso familiar era inferior a un salario mínimo (19 % por debajo de la línea de pobreza); 28 % de las mujeres no participaba en la adopción de decisiones sobre su salud, viéndose relegadas por sus parejas u otros familiares; 13 % correspondía a mujeres cabeza de familia (37 hijos huérfanos), y 52 % correspondió a causas directas, en tanto que 73 % era evitable. Conclusión. El análisis guiado por el enfoque de los factores sociales determinantes mostró un panorama en el que concurría una serie de características que posicionaban a estas mujeres en una condición de extrema vulnerabilidad, reafirmando que el trabajo para mejorar la salud materna debe orientarse no solo al manejo de los factores médicos sino de los problemas sociales estructurales.
