- Abstract
A endometrite é uma patologia de grande impacto na equideocultura, gerando altas perdas econômicas, pela redução do potencial de fertilidade da fêmea, e extensos gastos com os tratamentos. Essa inflamação do endométrio pode estar associada ou não a uma infecção bacteriana, fúngica ou a outros fatores como má conformação genital, deficiência nos mecanismos imunológicos, inseminação artificial, período pós-cobertura do garanhão ou pós-parto. A maioria das endometrites causadas por fungos possuem caráter crônico e incidência maior em éguas suscetíveis a endometrites persistentes. Os tratamentos dividem-se em convencionais e alternativos, ambos visando eliminar o agente causal e garantir a integridade reprodutiva do animal. Dessa forma, o presente estudo tem como objetivo relatar um caso de endometrite fúngica em uma égua doadora de embriões, da raça Quarto de Milha, 21 anos de idade, com histórico de falha reprodutiva, apresentando secreção vaginal mucopurulenta e presença de líquido ecogênico intrauterino no exame ultrassonográfico retal. Com base nos achados clínicos e anamnese, a suspeita era de endometrite e a mesma foi confirmada através da realização da cultura, indicando a presença do fungo Aspergillus spp. Primeiramente instituiu-se o tratamento convencional, com água oxigenada 10%, iodo povidine 2% e vinagre 5%, todos diluídos em solução de ringer lactato, com o objetivo de esterilizar e limpar o ambiente intrauterino. No entanto a terapia empregada não obteve sucesso, optando-se pela utilização do óleo ozonizado no momento do diestro, período onde a cérvix da égua se encontra fechada. Foi realizada a infusão de 60 ml de óleo ozonizado (30 ml em cada corno) com o auxílio de uma pipeta flexível, e aplicado PGF2 alfa a fim de dilatar a luz da cérvix e expulsar todo o conteúdo. Passadas 24 horas após a infusão do óleo, realizou-se a avaliação ultrassonográfica para constatar a eficácia do tratamento e uma massagem uterina vigorosa. Após 48 horas da infusão do óleo ozonizado, efetuou-se a lavagem uterina com ringer lactato com remoção completa dos 16 litros utilizados, onde o último lavado empregado retornou límpido. Após o término do tratamento, o útero permaneceu límpido, constatado pela ultrassonografia, e voltou a ciclar. Dessa forma, aguardou-se o cio subsequente e foi feita a inseminação artificial com sêmen fresco, obtendo prenhez positiva. Já é bem estabelecido na literatura que a molécula de ozônio possui grande ação antimicrobiana e antifúngica, rompendo a membrana celular dos microrganismos patogênicos e estimulando a produção dos linfócitos e monócitos para combatê-los. Ademais, o óleo de girassol ozonizado é um método de grande valia por possuir baixa resistência, fácil aplicação e um bom custo-benefício. Conclui-se que a sua utilização intrauterina, após o tratamento convencional realizado, proporcionou melhora do quadro clínico da paciente, sendo eficiente na recuperação de fertilidade em éguas com endometrite fúngica.
