- Abstract
Quadros clínicos de diarreia estão entre as principais afecções que acometem os equinos. Os agentes patogênicos mais comuns são a Salmonella spp., Clostridium difficile, Clostridium perfringens e o coronavírus. Por ser um sinal clínico inespecífico, a diarréia pode progredir para um prognóstico desfavorável devido ao intervalo de tempo entre os primeiros sinais e a intervenção. O objetivo deste trabalho é relatar um quadro clínico de diarreia e a conduta terapêutica, juntamente ao diagnóstico histopatológico de tiflocolite necrohemorrágica fatal em um equino. Foi admitido (D1) no Hospital Veterinário de Grandes Animais do IFSULDEMINAS - Campus Muzambinho, um cavalo da raça Mangalarga Paulista, de 13 anos, pesando 370 kg, com histórico de diarreia subaguda e emagrecimento progressivo havia 10 dias. O animal já havia sido avaliado e medicado por um médico veterinário externo. O mesmo receitou antibioticoterapia com sulfadiazina e trimetoprim (0,06 ml/kg, IM, SID por 7 dias), antinflamatório, suplementação nutricional, protetor hepático e gástrico. Ao chegar ao hospital, realizou-se o exame clínico geral, sendo solicitados hemograma, PPT e fibrinogênio, análise do perfil hepático e renal, verificando-se leucocitose acentuada por neutrofilia e hiperfibrinogenemia. Devido ao quadro de desidratação, realizou-se fluidoterapia ao longo da internação. Ademais, administrou-se escopolamina, lidocaína em infusão contínua e flunexin meglumine em D1. Não foi observada melhora no quadro no primeiro dia, sendo realizada a troca da antibioticoterapia pelo metronidazol via oral. Na tentativa de restabelecimento da microbiota, realizou-se transfaunação fecal (100 g VO, dose única) em D2, entretanto, o animal veio a óbito em D3. Na necropsia foram identificadas lesões macroscópicas compatíveis com tiflocolite necrohemorrágica, confirmadas ao exame histopatológico, o que levou ao choque endotoxêmico e posteriormente à morte. Com o histórico, sinais clínicos, piora com a terapêutica prévia e histopatologia, o animal recebeu o diagnóstico presuntivo de clostridiose. Na literatura é amplamente difundido que animais tratados com antibioticoterapia inadequada possuem chances de sobrevida menores do que animais não tratados ou tratados adequadamente. A utilização de sulfadiazina em casos de clostridiose pode promover o desenvolvimento elevado do agente patogênico devido a sua baixa sensibilidade ao princípio ativo, ao contrário do metronidazol, que é a opção inicial de escolha à doença. No caso relatado, não observou-se melhora clínica com a troca da antibioticoterapia devido à severidade do quadro. Assim, destaca-se a importância da definição de diagnósticos diferenciais para evitar a piora do paciente. Conclui-se que em casos suspeitos de diarreia bacteriana, a escolha da antibioticoterapia juntamente à ausência de diagnóstico laboratorial confirmatório podem influenciar consideravelmente o quadro clínico, alterando diretamente o prognóstico.
