The article discusses a pilot project in Washington state that used two theoretical models, the chronic care model (CCM) and the model for improvement, to address tobacco use in health care delivery organizations. Topics include an overview of the purpose of the study, which was the promotion of tobacco cessation-related changes within a health care system, and effectiveness of both models.
