- Abstract
The article discusses national reporting and learning system data on patient safety from the British National Patient Safety Agency (NPSA). According to the report, between April and June 2006, 176,857 patient safety incident reports were received. The majority of such reports that no harm occurred to patients. About 8% of incidents reported involved treatment, procedure, medication, access, administration, transfer and discharge incidents.
