Discontinuation of Multiple Psychotropic Drugs for Bipolar Disorder May Cause Reversible Encephalopathy.

- Abstract
Objective: Discontinuation of antidepressants is characterized by various somatic and psychological symptoms, and most of which are usually mild and transient. Method: We have experienced a case of bipolar disorder, who had been administered multiple antidepressants including tri- cyclic antidepressant (TCA), two types of selective serotonin reuptake inhibitors (SSRIs), serotonin noradrenaline reuptake inhibitor (SNRI), and mood stabilizer concurrently, and in whom after the discontinuation of these drugs, severe disturbance of consciousness and autonomie nervous system occurred. Result: After the conservative therapy, severe encephalopathy completely disappeared. The follow-up findings using magnetic resonance imaging (MRI) were compatible with reversible encephalopathy. Conclusion: Our case suggested that abrupt discontinuation of multiple psychotropic drugs for bipolar disorder may cause encephalopathy, and clinicians should consider the interactions of bipolar disorder related drugs and prescribe as simple as possible.
