- Abstract
Introducción. La osteogénesis imperfecta (OI) es un trastorno congénito caracterizado por fragilidad ósea causada por mutaciones en los genes que codifican el procolágeno tipo I (COL1A1, COL1A2). Al menos 4 tipos han sido descritas. La OI tipo I es la más común y menos severa de todas, la tipo II es letal, la tipo III causa deformidad severa, 50% de estos pacientes tienen fracturas in utero, y la tipo IV es muy rara y cursa con discreta angulación y acortamiento de las extremidades. Actualmente no existe un tratamiento específico para esta entidad, y el objetivo principal es disminuir o evitar la incidencia de nuevas fracturar, prevenir deformidades y tener mejor función de las extremidades. El uso del pamidronato ha sido descrito desde el año 2000 por Plotkin y col. Caso clínico. Presentamos el caso de un recién nacido masculino con OI tipo III. El diagnóstico se sospechó desde la semana 33 de edad gestacional. Un ultrasonido genético reveló micromelia y fracturas del fémur y tibia. Al nacimiento presentó múltiples fracturas en ambos húmeros, fémures y costillas. El paciente requirió la administración de tramadol para el control del dolor y al tercer día de vida recibió su primer ciclo de pamidronato en infusión de 4 horas por 3 días. La primera dosis fue calculada a 0.5 mg/kg y la segunda y tercera a 1 mg/kg. Presentó crisis convulsivas por hipocalcemia, las cuales cedieron al administrar gluconato de calcio. El paciente tuvo menos dolor y toleraba más los movimientos pasivos, incluyendo la palpación y movimientos activos, no presentó fracturas nuevas y fue egresado a los 30 días de vida. Actualmente ha recibido tres ciclos de pamidronato y se ha administrado calcio en infusión al mismo tiempo para evitar hipocalcemia. Ha aumentado su estatura y tiene mejor interacción con sus padres. Conclusiones. La literatura disponible demuestra que el pamidronato incrementa la densidad ósea, reduciendo las fracturas y el dolor en los pacientes con OI. En nuestro caso el uso del pamidronato fue seguro; actualmente el paciente tiene 6 meses y su calidad de vida ha mejorado con este tratamiento.
