Perturbation of rat renal tubule transport of the organic cation amantadine in recent onset streptozotocin-induced diabetes and in uninephrectomy.

- Abstract
The effects of early-stage diabetes mellitus and uninephrectomy on the renal tubule transport of amantadine were investigated. Kidney tubules were isolated from streptozotocin-induced diabetic (± insulin treatment), uninephrectomized, and control male Sprague-Dawley rats. There were no differences in the K[sub m] of amantadine uptake in renal proximal and distal tubules for the imposed treatments compared with control values. V[sub max] for amantadine uptake in the proximal tubules of diabetic and uninephrectomized rats was higher than the respective control (P < 0.05). V[sub max] for insulin-treated diabetic rats was similar to control values but was lower than that for untreated diabetic rats (P < 0.05). V[sub max] for distal tubule uptake was not altered by any treatment. Structure-activity studies demonstrated that bicarbonate-dependent amantadine uptake was inhibited by glycolate and lactate, but not by propionate or α-, β-, or γ-hydroxybutyrate. Early stage streptozotocin-induced diabetes mellitus and uninephrectomy induced changes in the kidney that resulted in a similar selective increase in proximal tubule amantadine uptake. These data represent the first description that experimentally induced diabetes mellitus and uninephrectomy modulate the function of the renal tubule organic cation (amantadine) transport system. Both interventions represent potential models in which phenotypic modulation of the renal elimination of organic cationic drugs may be achieved and studied.Key words: amantadine, organic cation transport, renal tubules, diabetes, uninephrectomy.On a examiné les effets du diabète sucré au stade précoce et ceux d'une uninéphrectomie sur le transport tubulaire rénal d'amantadine. On a isolé des tubules rénaux de rats Sprague-Dawley mâles rendus diabétiques par la streptozotocine (± traitement insulinique), uninéphrectomisés et témoins. La valeur de K[sub m] pour la capture d'amantadine dans les tubules proximaux et distaux n'a pas différé dans les traitements imposés comparativement à celle des rats témoins. La valeur de V[sub max] pour la capture d'amantadine dans les tubules proximaux des rats uninéphrectomisés et diabétiques a été supérieure aux valeurs témoins respectives (P < 0,05). La V[sub max] des rats diabétiques traités à l'insuline a été similaire à la V[sub max] témoin, mais elle a été inférieure à celle des rats diabétiques non traités (P < 0,05). La V[sub max] des tubules distaux n'a été modifiée par aucun traitement. Les études structure-activité ont démontré que le glycolate et le lactate ont inhibé la capture d'amantadine dépendante du bicarbonate, ce qui n'a pas été le cas pour le propionate ni pour α-, β-, et γ-hydroxybutyrate. Le diabète sucré induit par la streptozotocine et l'uninéphrectomie ont entraîné des modifications rénales qui ont provoqué une augmentation sélective similaire de la capture d'amantadine dans les tubules proximaux. Ces résultats montrent pour la première fois que l'uninéphrectomie et le diabète sucré induit expérimentalement modulent la fonction du système de transport du cation organique (amantadine) dans les tubules rénaux.
