- Abstract
Objective: Determination of CDT by various analytical methods to find the most suitable alternative for the originally used HPLC system. Type: Methodical study. Material and methods: 140 samples of patients from various specialized ambulances of the Moravian-Silesian Region with suspicion of alcohol abuse were included in the study. The CDT assay was performed according to validated procedures on the analytical systems: Variant (HPLC, Bio-Rad), MINICAP (Capillary Electrophoresis, Sebia) and BN ProSpec (Immunonephelometry, Siemens Healthineers). For statistical analysis, program Excel and MedCalc Version 16.1.2 were used. Results: Determination of CDT levels on analytical systems BN ProSpec, Variant and MINICAP, were verified. We evaluated the precision of the measurement under repeatability conditions, the lowest coefficient of variation (CV) was obtained on BN ProSpec, (CV 2.17 - 5.10%), and simultaneously the smallest deviation from declared values of commercial control materials at both levels (0.38% and 0.86%). Using the Passing Bablok regression, the best conformity was detected between Variant vs. MINICAP (y = 0.466 + 1.128 x). BN ProSpec reported the largest proportional error among systems. Using the Bland-Altman differential graphs the growing values of Bias were found in all three mutual comparisons, where the BN ProSpec provides lower concentrations of CDT% at higher concentrations than other systems. Kappa statistics confirmed that the best diagnostic conformity between systems provides Variant vs. MINICAP (0.851), the worst between systems Variant vs. BN ProSpec (0.696). Conclusion: BN ProSpec showed the best results of precision and accuracy of measurement. Kappa statistics demonstrated a very good conformity between Variant and MINICAP, respectively BN ProSpec analyzers. Nevertheless, BN ProSpec is due to simple, quick and trouble-free determination the most suitable replacement for the existing previously used analytical system Variant.
