- Abstract
In the Brazilian public healthcare system, the selection of medications for the Basic Component of Pharmaceutical Services is the responsibility of the different municipalities. The aim of the present study was to analyze differences and similarities in the number of medications, medication groups and organization of the Municipal Essential Drugs List (MEDL) in the Laguna region of the state of Santa Catarina and compare these lists with the 2010 National Essential Drugs List (NEDL). A further aim was to determine the epidemiological and nosological profile of the region studied. A descriptive study was carried out with comparisons between the different MEDLs and between the MEDLs and NEDL, with the identification of the process of medication selection and health indicators in the database of the National Health System. The mean number of pharmaceutical preparations on the MEDLs was 155 (range: 71 to 276). In the comparison of MEDLs with the 2010 NEDL, concordance ranged from 42.2 to 79.9%. The main criteria for the selection of medications were presence on the NEDL (n = 13), cost (n = 11) and requests from prescribers (n = 11). Only five cities had a Pharmacy and Therapeutics Committee. The main causes of death in the municipalities were cerebrovascular diseases, diabetes mellitus and acute myocardial infarction. Neighboring municipalities with similar epidemiological and nosological profiles differed widely regarding the qualitative and quantitative composition of the MEDLs as well as discordance with the NEDL. The findings demonstrate difficulties in the planning and execution of municipal pharmaceutical services in compliance with national guidelines and standards.
