- Abstract
The article discusses treatment of patients with long-term conditions (LTCs) in the 21st century that requires a shift in mindset, a different approach to prescribing, and a new kind of care. It is said that self-care plays a role in the management of chronic conditions. Lifestyle advice is considered a significant part of the role played by community nurses with their patients. Treatment of an LTC or several LTCs involves controlling a person's individualised symptoms.
