COMPARISON OF THE SHORT-TERM EFFECTIVENESS OF GRID LASER PHOTOCOAGULATION INTRAVITREAL BEVACIZUMAB AND INTRAVITREAL TRIAMCINOLONE THERAPIES IN DIFFUSE DIABETIC MACULAR EDEMA.

- Abstract
OBJECTIVE: We aimed to compare the short-term effectiveness of grid laser photocoagulation (GLP), intravitreal bevacizumab (IVB) and intravitreal triamcinolone (IVTA) therapies in diffuse diabetic macular edema (DDME) treatment MATERIAL AND METHOD: The records of 44 patients who had been diagnosed with DDME and performed GLF, IVB or IVTA treatment between 2011 and 2012 in our retina policlinic were scanned retrospectively. The subjects were diveded three groups as GLF(n:15), IVTA(n=14) and IVB (n=15). The best corrected visual acuity (BCVA), central macular thickness (CMK), intraocular pressure(IOP) change and macular sensitivity(MS) levels before treatment, 1 month after treatment and 3 months after treatment were compared. HbA1c level was also eveluated. RESULTS: There was not a signifi cant IOP change in a group during followings (p=0.887, p=0.283, p=0.822). There were not a statistically important change in BCVA and CMK in GLF Group (p==0.102, p=0. 281). Also statistically unimportant decrease in MS was detected in GLF group (p=0.147). There were statistically important increase in BCVA and CMK but statistically unimportant increase in MS at fi rst and third months in IVTA group (p=0.006, p=0.005; p<0.05, p=0.272). There were statistically unimportant increase in BCVA and CMK at fi rst month (p=0.054, p=0.112) but statistically important increase at third months (p=0.006, p=0.02;) in IVB group. MS increase was also statistically important in IVB group (p=0.002). The response to the treatment was beter if the HbA1c level was lower than 7% in GLF group. But the response was beter if HbA1c level was higher than 7% in IVB group. The response to treatment was not affected by HbA1c level in IVTA group. CONCLUSION: In DDME IVTA and IVB treatment results were also succesful in short term besides the GLF treatment. We thought that if the HbA1c level is higher than 7% it will be rational and suitable to delay the GLF treatment or prefer the IVB or IVTA treatment until the metabolic control is maintained.
