The Impact of Mesenteric Defect Closure on Internal Hernia Rates: Considerations on Follow-Up Duration.

- Abstract
A recent article in Obesity Surgery conducted a systematic review and meta-analysis on the relationship between the closure of mesenteric defects and the incidence of internal hernias after laparoscopic Roux-en-Y gastric bypass (LRYGB). The study found that closing mesenteric defects during LRYGB resulted in significantly reduced rates of internal hernias and reoperation for small bowel obstruction. However, the meta-analysis also revealed significant heterogeneity in the odds ratios for internal hernia rates among the included studies. To further explore this heterogeneity, the authors conducted a meta-regression analysis using the follow-up duration reported in each study. The analysis showed a correlation between the effect size and the duration of follow-up, suggesting that the protective benefit of mesenteric defect closure is more evident in the long-term follow-up. This finding emphasizes the importance of longer-term follow-up in studies examining this clinical question.
