- Abstract
It became clear in the 1980s from studies of individual antiretrovirals (ARVs) that single-drug therapies with nucleoside reverse-transcriptase inhibitors provided only temporary benefits to persons with HIV, so we turned our attention to studying ARV combinations, initially in vitro and then in the clinic. When it became clear a few years later that the human immunodeficiency virus type 1 (HIV-1) caused AIDS, we redirected our efforts towards understanding the pathogenesis and possible treatment of HIV infections. ACTG 175 convincingly showed that (1) antiretroviral therapy not only inhibited HIV replication but also saved lives, (2) 2 ARVs (zidovudine and didanosine) were better than 1 (zidovudine alone), and (3) measuring HIV ribonucleic acid (RNA) in blood was a valuable surrogate marker for clinical endpoints [[22]]. These results opened the door to many subsequent ARV combination clinical trials that changed the landscape of HIV therapy for decades to come.
