- Abstract
O plasma rico em plaquetas (PRP) é oriundo do sangue total, possuindo uma excelente concentração plaquetária. O PRP é de grande importância devido a sua notável produção de fatores de crescimento, promovendo o recrutamento de outras plaquetas, leucócitos e proteínas plasmáticas, estimulando a angiogênese, neovascularização, permeabilidade vascular, aumento da síntese de colágeno e proliferação de fibroblastos, contribuindo para a cicatrização de feridas cutâneas. O uso do PRP homólogo pode tornar-se extremamente interessante em animais debilitados, desidratados ou que sofreram episódio recente de hemorragia, necessitando deste tipo de tratamento, como também em animais neonatos, onde seu aporte sanguíneo é de extrema necessidade, possibilitando desta forma a utilização desta terapia quando necessário, com maior segurança e qualidade na técnica utilizada. O propósito deste estudo foi avaliar o efeito clínico e a qualidade cicatricial do uso de PRP em gel homólogo e autólogo em feridas induzidas cirurgicamente em equinos. No estudo foram selecionados 6 equinos hígidos, sendo que 5 destes foram submetidos à realização de feridas de forma cirúrgica. Em cada animal foram realizadas três lesões, as quais foram tratadas de diferentes formas. O grupo 1 obteve tratamento com PRP homólogo em gel, o grupo 2 recebeu o tratamento com PRP autólogo em gel e o grupo 3 pertenceu ao controle, recebendo como tratamento a solução fisiológica. O 6° animal foi o doador, fornecendo sangue para a produção do PRP homólogo. A fabricação do PRP autólogo e homólogo realizou-se com a mesma metodologia e, em seguida, realizou-se a contagem celular por meio de um analisador hematológico automático. As feridas foram avaliadas conforme sua apresentação macroscópica e submetidas à análise no programa ImageJ para quantificar o grau de evolução do processo cicatricial da ferida. Todas as lesões foram avaliadas nos dias 0, 15 e 30, juntamente à colheita de material para realizar biópsias, avaliando a qualidade da cicatrização. Não houve diferença estatística na diminuição da área da lesão aos 15 e 30 dias de tratamento (p > 0,05), embora visualmente os grupos tratados com a obtenção do PRP de forma heterogênea possuíam aparência reduzida comparados ao grupo controle. Conforme a inspeção macroscópica, não observou-se contaminação bacteriana e nem mesmo produção de secreção, e a produção de tecido de granulação foi inexistente. O uso de gel de PRP pode diminuir o tempo de cicatrização de feridas em poucos dias em equinos. O PRP homólogo, além de seguro, foi ligeiramente superior ao PRP autólogo na avaliação macroscópica e muito semelhante na avaliação histopatológica, proporcionando excelente qualidade cicatricial. Conclui-se que o uso do PRP homólogo é uma ótima alternativa para ser utilizado em pacientes neonatos, que estejam com trombocitopenia ou debilitados.
