- Abstract
Objective: To investigate the effect of ulinastatin combined with Shenmai injection in the treatment of early acute lung injury (ALI) after trauma and its influence on serum tumor necrosis factor-α(TNF-α) and interleukin-6 (IL-6) levels. Methods: A total of 200 patients with early ALI after trauma, who were admitted to Nongken Sanya Hospital of Hainan Province from June 2012 to June 2016, were collected and randomly divided into conventional group(n=100) and experimental group(n=100). The conventional group was given conventional western medicine treatment, while the experimental group was added ulinastatin and Shenmai injection on the basis of the treatment of conventional group. The clinical efficacy, intensive care unit (ICU) days, total length of stay in hospital and the incidence of acute respiratory distress syndrome (ARDS) of the two groups were compared. The blood gas indexes of the two groups before and after treatment were detected; and the levels of serum TNF-α and IL-6 of the two groups before treatment and 1 d, 3 d, and 7 d after treatment were detected by enzyme linked immunosorbent assay method. Results: The total effective rate (88.00%) of the experimental group was significantly higher than that(75.00%) of the conventional group,the difference was statistically significant (P<0.05). The ICU days, total length of stay in hospital and the incidence of ARDS in the experimental group were lower than those in the conventional group, the differences were statistically significant (P<0.05). The partial pressure of artery(PaO2) and oxygenation index (PaO2/FIO2) in the experimental group were higher than those in the conventional group, the differences were statistically significant (P<0.05). The levels of serum TNF-α and IL-6 in the experimental group were lower than those in the conventional group 3 d and 7 d after treatment, the differences were statistically significant (P<0.05). Conclusion: Ulinastatin combined with Shenmai injection has a good clinical efficacy in the treatment of early ALI after trauma, which can effectively reduce oxidative stress, improve pulmonary microcirculation and cut down the duration in ICU and hospital, improve pulmonary oxygenation function, and reduce the serum TNF-α and IL-6 levels.
