- Abstract
Cerebral computerized tomography (CT) and magnetic resonance (MR) after excellent anatomic information to the diagnosis of patients with depression and significant cognitive deficits, while single photon emission computerized tomography (brain SPECT) enables us to obtain functional information through the measurement of the regional cerebral blood flow (RCBF). Material: Twenty-three patients, 9 men (39.1%) and 14 women (60.9%), mean age 61 years, with the diagnosis of depression and significant cognitive deficit without complying with Dementia criteria, according to the diagnostic codes GMS-HAS and DSM-III-R. Ten healthy individuals of similar sociodemographic characteristics and with no previous history of psychiatric or neurologic disorder nor drug abuse. Methods: Cerebral computerized axial tomography (CT) and single photon computerized tomography were performed systematically on subjects of over 65 with a MEC < 24 and of under 65 with a MEC>27. Results: Six cerebral perfusion patterns were established for the patient group. The most commonly observed of these was pattern C (39.13%), temporal and/or bilateral parietal perfusion. Conclusions: 1. Cerebral SPECT is a useful instrument as a complement to the differential diagnosis of dementia and depression in the elderly patient. 2. Patients with degenerative dementia display a reduction in the regional blood-flow in the temporoparietal and parietal cortex in basal SPECT. Depressed patients display overall hypoperfusion in basal SPECT, which is more evident in the frontal and temporal regions and in the right hemisphere
