- Abstract
Objectives: One of the most frequent locomotor diseases in childhood is the developmental dysplasia of the hip (DDH). The definition has been changed in the last 20 years; it refers to a continuum of abnormalities in the immature hip that can result in subluxation or dislocation of the femoral head [1]. The term developmental dysplasia of the hip has been suggested by the European Paediatric Orthopaedic Society. By ultrasound (US) early stages can be detected thus treatment can be started prior the clinical signs. Our aim was to assess the effectiveness of physical examination and the early universal ultrasound screening and to evaluate the risk factors. Methods: A retrospective study of universal hip screening of all mature neonates (≥37 gestational week) was conducted in 2017 at the Department of Obstetrics and Gynaecology, University of Szeged. Altogether 1446 newborns had clinical examinations and hip ultrasound within the 1-4th days of life, and positiv cases were controlled every 4 weeks. Prevalence of DDH, risk factors were evaluated. Results: At the 1st US, 85 of the examined 1446 neonates (5.87%) were found to be positive, with dysplastic hip at least at one side, 80% of them were female. In 81.9% of the cases with US positivity, physical examination was negative. Regarding the risk factors, in 25 cases (29.4%) only female gender, in 5 (5.9%) positive family history and in 8 neonates (9.4%) breech presentation were the only risk factors. In our study, all hips became negative by the forth control examination due to spontaneous changes or the therapy. Conclusions: Our results have shown the importance of universal hip US screening, however further studies are needed regarding screening in such an early phase. Universal US hip screening before 6 week of life could be a proper solution.
