- Abstract
A neuro-risk child is any child who has an underlying history of any neuro-risk factor and/or child who, during the examination, exhibits symptom of deviation from normal development. When classifying neuro-risk children, the most commonly used is the division into highly neuro-risk and low neuro-risk children. The degree of neuro-risk determines the degree of diagnostic and therapeutic procedures. Neurodevelopmental outcome depends on the interaction of the child and the environment, the existing brain damage (type, extent and localization) and the compensatory processes of maturation and plasticity of the brain. In the treatment of high-risk children, two basic techniques of medical gymnastics are nowadays most commonly used: Neurodevelopmental Therapy (NDT) - according to Karl and Bertha Bobath and kinesiological therapy by Vaclav Vojta. NDT is a holistic approach that deals not only with muscle function problems and with sensomotoric development problems. Includes the whole person, perceptive and cognitive weakness and emotional, social and functional problems in everyday life. The treatment is closely related to the patient's wishes and needs. Basic principle of the NDT treatment is: by inhibiting the control of an abnormal motion pattern and simultaneously by facilitating automatic postural reactions (Righting reactions, Equilibrium reactions) combined with various stimulation techniques, we reduce the dysfunctional abnormal postural tone and transmit to the child various sensory motor experiences in functional and purpose-directed activities is the same. The goal of the therapy is to provide the child with a more accurate sensomotor experience as a basis for building a more normal development. Treatment outcome is significantly better in younger children when abnormal reactions are less dominant. Only the activity of normal motion patterns prevents the dominance of abnormal reactions. Numerous studies on NDT efficiency have been published. The difficulty in proving NDT efficiency stems from Bobath's holistic approach, covers all areas of everyday life, and includes counseling and training for caretakers, equipment and other modalities and is severely measurable. Lack of evidence does not mean that empirical strategies that work and beneficial aspects of Bobath's approach have to be rejected. On the contrary, the challenge is to provide evidence of their effectiveness.
