- Abstract
The author discusses the blood transfusion process and the importance of importance of error reporting in the process. The author mentions strategies to minimize and prevent possible risk for fatal acute hemolytic transfusion reactions (HTR) due to hemolytic disease of the newborn (ABO)-incompatible blood. Among these strategies were the use of evidence-based transfusions, implementation of patient identification process, and the identification and reporting of errors.
