- Abstract
The article discusses the treatment options for patients with single-nodule hepatocellular carcinoma (HCC), including local ablation, surgical resection, and liver transplantation (LT). The study found that a significant proportion of patients who underwent local ablation or surgical resection experienced HCC recurrence beyond LT criteria, leading to a poor prognosis. The Barcelona Clinic Liver Cancer (BCLC) staging system recommends local ablation or surgical resection for patients with preserved liver function and a single-nodule HCC, but these treatments have high recurrence rates. LT is associated with lower recurrence rates and higher survival rates, but the limited availability of donors may prevent some patients from accessing this treatment. The article suggests that upfront liver resection should be considered for patients with a low risk of recurrence, while LT should be offered as the first-line therapy for patients with a high risk of recurrence. The use of immunotherapeutic agents before or after LT is still being studied, and personalized treatment decisions should be made based on individual patient factors and available resources.
