- Abstract
Introduction According to the literature, 0.5 % of patients with spontaneous pneumothorax have an associated hemothorax. The management should be fast and adapted to each case, according to the etiology. The aim of our study is to analyze the etiologies of spontaneous hemothorax while reporting the results of our management. Methods We conducted a retrospective study between January 2010 and July 2016 in the thoracic surgery department of CHU Hassan II of Fez, it was carried on seven patients treated for spontaneous hemothorax. Results All the patients were males, aged between 17 to 75 years with a mean age of 41.85 years. Chronic smoking history was present in five patients. One patient was anticoagulated for mitral valve disease operated two years before. The hemothorax was complicated by a spontaneous pneumothorax in five patients. In two patients, the hemothorax was isolated. Dyspnea and chest pain was the admission patterns in all patients, two patients were admitted in hemorrhagic shock. Chest X-ray showed a fluid and air level, in five cases and pleural effusion in two other patients. The abnormalities were found on the right side in four cases out of seven. Five patients have had exploratory surgery, four after the failure of a thoracic drainage, and one for whom the surgery was indicated at once. Hemostasis was obtained spontaneously after clot removal in trois patients and no bleeding source was objectified. The postoperative recoveries were smooth, we have not recorded bleeding recurrence after a mean duration of 17 months. Conclusion In case of massive spontaneous hemothorax responsible for hemorrhagic shock, surgical indication should suffer no delay.
