- Abstract
Prescribing errors are a significant concern in healthcare, with avoidable adverse drug events causing deaths and costing the NHS millions of pounds each year. The EQUIP report identified various factors contributing to prescribing errors, including high workload and inadequate education. However, the narrative surrounding prescribing education may be hindering rather than helping. Fear of making errors and being blamed for them is prevalent among medical students and doctors, which can lead to a culture of blame and hinder learning. Shifting to a more positive and constructive approach, where errors are openly discussed and feedback is given without fear of blame, may be more effective in reducing prescribing errors.
