PRESENÇA DE AMASTIGOTA DE LEISHMANIA SP. EM SANGUE PERIFÉRICO DE FELINO.

- Abstract
A Leishmaniose Visceral é uma zoonose endêmica no Brasil, considerada de risco para a saúde pública e subdiagnosticada em felinos, pois são frequentes os casos de animais com sintomas ausentes ou inespecíficos. Desse modo, é importante sempre incluir a leishmaniose como diagnóstico diferencial nessa espécie, a fim de melhor esclarecer o ciclo da doença, reduzir os riscos de falsos-negativos e proporcionar maior proteção aos tutores de gatos. A foto retrata uma amastigota de Leishmania sp. (seta) no interior de um neutrófilo no sangue periférico de um felino. Um gato de 3 anos, não castrado, foi atendido após resgate com suspeita de atropelamento. No exame físico o paciente apresentou anorexia, adipsia, aquesia, vocalização, hipotermia, caquexia, linfadenomegalia, onicogrifose e mucosas hipocoradas e ictéricas. No hemograma foi observado anemia moderada normocítica hipocrômica, hiperproteinemia por hiperglobulinemia, leucocitose por neutrofilia e linfopenia com neutrófilos tóxicos, linfócitos reativos e formas amastigotas de Leishmania sp. Na bioquímica sérica foi constatado aumento da atividade da alanina aminotransferase (ALT) e azotemia. Desse modo, observamos anemia causada por inflamação acentuada e não responsiva decorrente do curso crônico da leishmaniose, o que resulta na redução do ferro disponível para a síntese de hemoglobina. Tal condição pode ainda ser agravada pela anorexia, caquexia e possível doença renal crônica por deposição de imunocomplexos que o paciente apresenta. O aumento da atividade de ALT, associado à coloração das mucosas, sugere icterícia hepática. A linfopenia com presença de linfócitos reativos pode indicar estresse crônico, coinfecções não detectadas e aumento da resposta imune humoral para produção de imunoglobulinas. O diagnóstico definitivo do caso foi possível apenas a partir da visualização do parasito no esfregaço sanguíneo, o que destaca a importância do exame citológico no hemograma quando realizado por um profissional capacitado.
