- Abstract
Introducere. Identificarea femeilor care pot dezvolta sau prezintă cancer cervical este o preocupare mondială majoră, concretizată prin eforturi susținute de implementare a unor diferite metode de screening și triaj al leziunilor cervicale precanceroase (LCP). Scopul studiului este de a sublinia caracteristicile biotehnologiilor avansate și capacitatea acestora de a formula un diagnostic precoce corect în măsură să impună abordări terapeutice adecvate. Materiale și metodă. Studiul nostru cuprinde rezultatele mai multor trialuri și studii observaționale publicate în bazele de date internaţionale (PubMed, Medscape, Scopus) de-a lungul ultimilor 10 ani. Culegerea și analiza datelor au condus la identificarea biotehnologiilor implementate, la compararea eficienței acestora din punctul de vedere al sensibilităţii în detecția și triajul LCP și la prezentarea posibilităților diferite de accesibilitate a femeilor de pe glob la aceste tehnici de diagnostic. Rezultate. Testul citologic Babeş-Papanicolau se impune prin specificitate crescută de detecție a cazurilor care nu sunt susceptibile de a avea LCP sau cancer cervical. Raportat la metodologia de prelucare, nu există diferențe calitative între metoda citologiei uscate sau în mediu lichid. În ceea ce privește metodologia existentă pentru genotiparea virusului papiloma uman (HPV), sunt aplicabile trei metode: acid hybridization assays, signal amplification assays şi nucleic acid amplification. Testele imunohistochimice (IHC) de identificare singulară a ciclinei p16 ink4a sau asociată cu detectarea ciclinei Ki67 (dual test) cresc acuratețea diagnosticului. Identificarea oncogenelor E6 și E7 tranșează diagnosticul. Concluzii. Testul Babeş-Papanicolau este o metodă scumpă de diagnostic, are un grad ridicat de subiectivism în evaluarea citologiei și nu conferă posibilitatea triajului dintre leziunile ASC-US și L-SIL. Singură, genotiparea HPV nu permite diferențierea dintre infecția HPV tranzientă și cea progresivă. Testarea imunohistochimică pentru identificarea ciclinei p16ink4a și/sau dual test sunt acceptate universal în identificarea leziunilor cervicale progresive. Metodele de screening al LCP sunt adoptate diferit de programele naționale, în raport cu disponibilitatea financiară a fiecărei țări.
