- Abstract
Introduction: The act of notifying needs to be seen as an opportunity for health professionals to report adverse health events, in order to promote greater vigilance among service providers and organizations, allowing improvements in patient safety and learning from the failures found. The present study aimed to report the implementation process of a system for operationalizing the process of reporting health events and/or incidents in a reference hospital for cancer treatment. This is an experience report, carried out at the Aldenora Bello Cancer Hospital, where actions were taken to promote the notification of incidents related to patient safety by all team members. The most effective was the creation of the Qr-Code and its placement in strategic locations. Case report: There was an increase in the number of notifiers and greater diversity in events that affect the quality of care. The creation of the Qr-Code proved to be a simple and useful tool to encourage the reporting of adverse events and add it as a new communication channel for the hospital, keeping professionals away from judgment and punishment. Conclusion: Voluntary reporting needs to be understood as an opportunity to improve care in favor of patient safety and the construction of an organizational environment for learning from mistakes. It is necessary to constrain utopia by accepting the possibility of making mistakes, with the aim of achieving cultural transformation and adherence to safe health practices.
