- Abstract
V rámci sympozia společnosti Novartis na 9. konferenci Neurologie pro praxi v lednu 2022 představil prof. Vališ ofatumumab (Kesimpta®, Novartis) – nový přípravek pro léčbu relabující remitentní roztroušené sklerózy (RRRS). Tato monoklonální protilátka proti CD20, která navozuje depleci B lymfocytů, prokázala v klinických studiích v porovnání s teriflunomidem významné snížení počtu relapsů za rok o více než polovinu, téměř dvojnásobné snížení počtu Gd+ T1 lézí na MR, 5násobné snížení počtu nových nebo zvětšujících se T2 lézí za rok, o třetinu nižší riziko potvrzené progrese disability po dvou letech léčby a dosažení NEDA-3 téměř u 9 z 10 pacientů. Ofatumumab je k dispozici v autoinjektoru a pacient si jej po iniciálním dávkování podává 1x měsíčně sám doma. Prof. Vališ prezentoval i výsledky dokládající bezpečnost ofatumumabu srovnatelnou s teriflunomidem a kazuistiku 38leté pacientky s RRRS s masivním nálezem na RS, u níž ofatumumab po předchozí léčbě interferonem β-1a a dimethyl fumarátem minimalizoval výskyt relapsů a zastavil zhoršování disability. MUDr. Peterka se věnoval problematice sekundárně progresivní roztroušené sklerózy (SPRS), která v průběhu onemocnění postihne velkou část pacientů s roztroušenou sklerózou. Připomněl konsenzuální definici SPRS, která může pomoci jejímu časnému záchytu, což má dnes vhledem k dostupnosti schválené terapie klinický význam. Selektivní modulátor receptoru pro S1P1,5, siponimod (MAYZENT®, Novartis), je první perorální lék pro pacienty s SPRS. Působí jak v periferii, tak v CNS, a cílí na zánět i na neurodegenerativní procesy. V placebem kontrolované registrační studii EXPAND prokázal zpomalení progrese disability, příznivý vliv na kognitivní funkce a dobrý bezpečnostní profil. Od 1. února 2021 je siponimod v ČR hrazen ze zdravotního pojištění pro pacienty s aktivní SPRS.
